Helen, 64-year old lady

Helen is a 64-year-old lady who had a left MCA infarct. This affected the right side of her body as she presented with severe motor and sensory loss to her lower and upper limbs. Initially, she was unable to sit upright without support and had impaired sitting balance. She would lean heavily to her weaker right side due to lack of muscle activation and strength at her core and trunk. She spent a long period of rehabilitation in the hospital where she gradually regained movement to her right leg and arm. However, during the initial stages due to long periods of lying in bed, she developed a Grade 1 pressure injury on her sacrum. On discharge from hospital, she was able to mobilise with maximum assistance of one person for safety with standing and prompting for bringing attention to her right side when walking. She was prescribed the configura comfort chair to support her for the following reasons:

  • As she was quite a small stature, Helen was able to sit comfortably in the small configura comfort chair with a depth adjustment kit to reduce the depth of the chair.
  • She also required adjustment to reduce the width between the armrests, this allowed her to rest her weak right arm on to armrest to reduce subluxation at the shoulder. This also aided her in her transfer to push up easily from the left side when standing up.
  • The medium-rise setting was used for Helen as she found the standard rise setting pushed her forward too much and she had reduced core and balance when standing.
  • Due to reduced function in her right affected limb, the handset was moved to be positioned on the left side for her to use independently to raise her legs and recline backward as she wished. This gave her a level of independence between care calls for repositioning.
  • Her sitting balance and posture improved with increased strength and rehabilitation however during the evenings she would fatigue, and the lateral support wedges were positioned on her right side to prompt her to remain sitting upright and in alignment.

Due to her history of pressure injury, she was provided with the CushionAir alternating cushion to replace the standard cushion. This gave her a high level of pressure care and relief. She was also provided with a positioning guide to spend periods of the day in tilt in space and recline for redistribution of pressure.
