Configura Advance Chair

CLINICAL JUSTIFICATION

It is important to note that 2 people with the same diagnosis will present differently. The below clinical justifications are a guideline only of how the Configura Advance Chair is an appropriate seating option for different client groups. To be used always in conjunction with a comprehensive seating and risk assessment.

FEATURES

Allows the legs to remain bent when moving backward into tilt in space. This is suitable for clients who have shortened hamstrings, lower limb contractures and spasticity.

Avoids the risk of backrest position being adjusted incorrectly which could have detrimental effects on a person's posture.

Reduces the level of shear and movement in the hips when the backrest is reclined. Ideal for stability at the pelvis during movement of the chair.

Assists with standing transfers (ensure removal of footplate).

Leg rest elevation helps the user to relax and can help reduce fluid build-up in the legs used in conjunction with tilt in space.

The leg rest function has been designed to lengthen with elevation using body move technology. This helps to ensure the whole leg is supported by the leg rest in elevation.

Designed for people with contractures at the knees and shortened hamstring with reducing knee extension and therefore needing to be compensated within the leg rest angle of the chair.

Suitable for better access to the side of the body for sling positioning.

Castors are appropriate for transferring non ambulant users to access multiple areas in their environment and reduces the frequency of manual handling from one chair to another.

Armrests can be adjusted to slide in and out to accommodate width measurement of the person, this helps in encouraging correct neutral sitting position by avoiding leaning onto one armrest.

Backrest can be adjusted to slide forward and backwards to accommodate the depth measurement of the person in encouraging correct positioning of the pelvis in the seat and to avoid poor postures developing.

Footplate position slides up and down to manage height in supporting a person's feet correctly and avoiding foot drop.

The footplate is easily removed and designed so there is no need to change height each time. This allows assurance that the set up will remain as scripted.

Heat sensitive memory foam moulds to the user providing maximum support and pressure redistribution. Suitable for users who are at risk of pressure damage.

Powered alternating air cushion which provides pressure relief for those who have developed pressure damage. A row of cells of the cushion inflate and deflate alternately providing pressure offloading and encourages circulation in the area.

The pressure relieving fabric and visco cushioning is integrated within the length of the leg rest to prevent pressure injuries developing.

Each pillow is height adjustable and filling can be customised to the user. Offers mild to moderate postural support and is highly effective for users with a kyphosis or scoliosis.

The complex postural backrest integrates winged backrest with multiple angle settings to allow for varied postural support needs from high to complex posture management and correction.

The lateral supports can be adjusted along the length of the trunk for moderate postural support. It is designed in 2 parts, fitting to chair, and separate lateral, so when taking each lateral out it does not impact the original and scripted positioning.

Combines the benefits of the waterfall backrest with adjustability of the stuffing within each area of the backrest whilst also providing a higher level of lateral support.

Available to support pelvic stability and maintain suitable positioning. (Must not be used as a constraint.)

CLIENT SYMPTOMS

The use of the advance configura is common for clients with advancing dementia, intellectual or severe cognitive impairments. When cognition impacts a persons ability to initiate movement this can results in postural deformities and immobility. The mechanical advance chair is advised to use for these clients as there could be a risk of the handset being used in an unsafe manner. Make sure to guide the client as to the upcoming movements occuring in the chair to minimise any behavioural reactions to sudden tilt in space or backrest recline. The advance chair allows for a person with dementia to access social and communal areas of their home to maximise their engagement in meaningful activities known to greatly benefit a person with dementia.

The Advance Configura is ideal for clients who are unable to transfer in and out of their seating independently. This applies to those who require a hoist to transfer and for those who are using standing aids to transfer. The large castors allow the chair to move around the persons environment. The space below the frame of the chair is ideal for access for a hoist legs to get close to the chair for the transfer. Drop down armrests and the tilt in space features of the chair also assist in optimising a safe and successful transfer for immobile clients. In order to maintain or rehabilitate standing and mobility we recommend using the anterior tilt feature.

For clients who suffer from fatigue impacting on their postural abilities the configura advance feature will aid them to maintain more neutral and ideal seated postures. Maintain upright posture requires a high level of core muscle activation and a person with fatigue will benefit from the mechanisms of the chair to reduce this activity level. Use of tilt and space and leg rest elevation allows a person with fatigue to remain sitting for longer periods of time without contributing to their physical fatigue.

Fabric

The Configura Advance can be used very successfully for someone with high tone and spasticity affecting their range of motion of postural alignment. The configura advance allows a therapist to adjust the shape and configuration of the chair to the non reducible joints of a client to create ideal support and comfort.

The Configura Advance has multiple backrest support options for a person with low tone. Low postural tone can greatly impact a persons ability to maintain an upright seated posture so aggressive postural supports are often required to help aim towards more neutral alignment. The complex winged backrest and fixed lateral supports can be used along the trunk of a client to assist with this more corrective support. Tilt in space is also ideal for maintain the pelvis at the back of the chair and helping align the trunk in midline along the backrest.

The configura advance can help a person with poor sitting balance to achieve and maintain upright posture. Using the postural supports and tilt in space for postural alignment. This can also be used to gradually increase seated balance for someone post neurological event to slowly reduce the level of tilt and increase seating tolerance.

The Configura Advance can be used very successfully for someone with reduced lower limb range due to the true tilt in space feature. The leg rest stays at 90 degrees whilst the seat base tilts back. The leg rest can be adjusted to the angle the person can achieve in the knee extension to open up this joint, or for those with severe limitations to knee range the negative leg rest angle of less than 90 degrees can be used.

The Configura Advance allows for lower limb fluid drainage for those who have lower limb oedema. With the chair in tilt and space, leg rest elevation and backrest recline, the chair moves into a position where the legs are higher than the heart.

The Configura Advance has multiple backrest options for supporting a person with scoliosis. As this condition creates a fixed deformity of the spine into an S shape are role is to prevent this positioning from getting worse. This can be achieved with 3 points of control along the backrest with the lateral supports. This can be done using the adjustable lateral support backrest or the fixed laterals of the complex winged backrest.

The Configura Advance allows for optimal support and comfort for those with a kyphotic posture. It is important to create a backrest that allows a person thoracic spine to be immersed by the cushions and that also offloads the vulnerable spinal processors from pressure injuries. By positioning the waterfall cushions to be orientated vertically this creates a very comfortable backrest option for these clients. The cocoon backrest is also successfully used by removing some of the stuffing to accommodated for the spine. These backrest options used in conjunction with tilt in space assists in maintain a more upright posture whilst also allowing for eye gaze to meet midline for social interactions.

The Configura Advance can be used for people who sit in pelvic obliquity. The main aim of intervention is to build up the seat base to create a more even distribution of weight in order to reduce the risk of pressure areas. This can be done by adding foam wedges under the seta base fabric or by fitting in a customised seat cushion.

The Configura Advance allows for prevention and management of pressure injuries. The advanced chair accommodates for pressure care needs with a standard medium to high prevention cushion which is ideal for pressure redistribution through immersion and envelopment into the memory foam cushion. If a person has developed pressure areas and is at a very high risk of pressure injuries the cushion can be replaced to a higher level of pressure relief using the Cushion Air which provides the person with alternating air on a 10 minute cycle. The amount of time sitting out and other interventions like repositioning and weight shift should be considered for a person with pressure injuries.

The Configura Advance can accommodate a person with a foot drop. By opening up the angle of the footrest a person who has an angle of more then 90 degrees in a non reducible ankle join can be properly supported. There are 3 angles available and this helps to prevent the foot drop from getting worse.

The Configura Advance can be used successfully for people who have involuntary movements, tremors or spasms. By using the tilt in space feature a person with these movements would be much safer by reducing their risk of slipping forward and out of the chair. The frame of the chair can also withstand the movement of a person with severe involuntary movements i.e. for a person with Huntington's disease. The vapour permeable fabric also helps to minimise the risk of pressure injuries occurring with rubbing of the skin against the chair with these movements.

The configura advance can be used successfully for people who experience pain by providing them with multiple backrest and seat cushion option in order to find more optimal comfort level in seating.
